CYTOMEGA, Valganciclovir is used in adults to prevent infection with cytomegalovirus (CMV) that may occur after an organ transplant (heart, kidney, or pancreas). It is actually a prodrug for ganciclovir. After oral administration, it rapidly convert into ganciclovir by intestinal and hepatic esterases. CYTOMEGA is also used to treat CMV retinitis in people who have acquired immunodeficiency syndrome (AIDS).

MODE OF ACTION:
Valganciclovir is a prodrug of ganciclovir, which inhibits cytomegalovirus (CMV) replication by inhibition of viral DNA synthesis. Ganciclovir triphosphate is a competitive inhibitor of deoxyguanosine triphosphate incorporation into DNA and preferentially inhibits viral DNA polymerases more than cellular DNA polymerases. In addition, ganciclovir triphosphate serves as a poor substrate for chain elongation, thereby disrupting viral DNA synthesis by a second route.
INDICATIONS
It is used to treat cytomegalovirus (CMV) infection, prevent CMV disease in high risk kidney, heart, and kidney-pancreas transplant patients. Also in CMV retinitis in adult patients with AIDS, Herpes viruses in vitro, including herpes simplex virus types 1 and 2 (HSV-1, HSV-2), Symptomatic herpes simian B virus disease & Central nervous system infections.
DOSAGE
The dosing of valganciclovir, in adult patients with normal renal function, is 900 mg twice daily for 21 days during induction therapy followed by single daily dosing during maintenance therapy. Dose Adjustment: CrCl ≥60 mL/min: Usual dose (ie, 900 mg every 12 hours for induction; 900 mg once daily for maintenance) , 40-59 mL/min: 450 mg every 12 hours for induction; 450 mg once daily for maintenance, 25-39 mL/min: 450 mg once daily for induction; 450 mg every 48 hours for maintenance, 10-24 mL/min: 450 mg every 48 hours for induction; 450 mg twice per week for maintenance.
